The Certificate of Correction is found on the reverse side of the GOLD copy of the Notice of Violation. First offenders must sign this in the presence of a Notary Public and return it with all appropriate documentation to be excused from appearing at the scheduled ECB hearing. Repeat offenders MUST appear at the scheduled hearing to provide proof of correction.
