For our valued customers, we are reiterating the New York City Fire Department’s requirements for complying with your NYC Notice of Violation as written on the back of your Certificate of Correction.

IMPORTANT: The following instructions apply to FIRST OFFENSES ONLY. Repeat offenders MUST appear at a hearing on the date specified on the front of the Notice of Violation.
||All offenders must correct the violations cited on the Notice of Violation (NOV). Contact Modern Sprinkler to find out if we can help.
||All offenders must complete the Certificate of Corrections and have it notarized. Do not detach the certificate.
||Attach legible copies of any and all bills, receipts and/or other proof of compliance to this Certificate of Correction. Please refer to the correction requirements on the reverse of the Green copy and submit them along with the certificate (Gold copy). Also, use the extra copy of Modern Sprinkler’s letter as Proof of Compliance.
||Return the completed Certificate of Correction, with all appropriate documentation attached, to:

This must be completed no later than the compliance date indicated on the notice of violation.See the reverse side for return date.
||Respondents will be notified by mail whether or not their Certificate of Correction has been approved. If the certificate is NOT approved, you will be notified of the appropriate action to take.
||If it is impossible to correct any or all violations within the time indicated, you may be eligible for a stipulation, extending the time to correct. A request for stipulation extending compliance time must be made at the hearing and is subject to the approval of the Fire Department and the Environmental Control Board, (ECB). This applies only to Rule #12.
||If you properly certify that all violations have been corrected, you will be excused from appearing at the scheduled ECB hearing and no penalty will be imposed.
||If you FAIL to comply with item #7, or if you are a repeat offender, you will be required to attend the ECB hearing designated on the NOV and bring any and all proof of correction to the hearing.
||An appointment must be given within 35 days of the date of violation. Modern Sprinkler cannot make an appointment for Rule #20 unless we receive authorization from the customer.
Note: It is imperative that these instructions be complied with. Failure to do so can result in inconvenient hearing appearances and large fines. Modern sprinkler will help with any matters it can, but ultimately the customer will be held responsible.
